Dosing
Cytomegalovirus Infections 400 mg (two 200 mg tablets) orally twice daily with or without food; increase to 800 mg twice daily when coadministered with carbamazepine or rifabutin; increase to 1,200 mg twice daily when coadministered with phenytoin or phenobarbital.
Cytomegalovirus Infections (HSCT recipients >=30 kg) 480 mg once daily orally or IV infusion over 1 hour through 100 days post-HSCT; may continue through 200 days in patients at risk for late infection. Reduce to 240 mg once daily when co-administered with cyclosporine.
Cytomegalovirus Infections (Kidney transplant recipients >=40 kg) 480 mg once daily orally or IV infusion over 1 hour through 200 days post-transplant. Reduce to 240 mg once daily when co-administered with cyclosporine.
Cytomegalovirus Infections (Pediatric HSCT recipients 6 months to <12 years or >=12 years and <30 kg) Weight-based dosing once daily orally or IV infusion over 1 hour through 100 days post-HSCT; may continue through 200 days in patients at risk. Dose adjustment may be required when co-administered with cyclosporine.
Contraindications

- Co-administration with pimozide (risk of QT prolongation and torsades de pointes)
- Co-administration with ergot alkaloids (ergotamine, dihydroergotamine; risk of ergotism)
- Co-administration with pitavastatin or simvastatin when combined with cyclosporine (risk of myopathy or rhabdomyolysis)

Pharmacology
Maribavir is a benzimidazole riboside antiviral that competitively inhibits the protein kinase activity of human CMV pUL97, resulting in inhibition of phosphorylation of viral proteins required for CMV replication.
Letermovir inhibits the CMV DNA terminase complex (pUL51, pUL56, pUL89), which is required for viral DNA processing and packaging, thereby interfering with virion maturation.
